Here is a list of the Participating Insurance Providers at Premier Dermatology. As all insurance plans are different, we ask that you check with your plan to see if you need a referral prior to seeing us. All referrals, authorizations, and participation inquiries should reference Premier Dermatology’s national provider identification number (NPI) of 1700202298. If you have any questions regarding our participation with your plan, please call us or contact your insurance carrier at the phone number located on the back of your insurance card.
The packet provided by your insurance carrier will include detailed information about what is covered (or not covered) by your policy. For example, some insurance carriers may exclude benefits for removal of certain types of benign skin lesions. Our staff can assist you in determining if your services will be covered by your insurance. The specific benefits for policies can vary widely (even those issued by the same insurance carrier), however, so we may not always know the answer and you may need to contact your insurance carrier directly for verification.
Most policies require patients to pay a portion of their health care costs as an out-of-pocket expense. We are obligated, by contract, to collect co-payments, co-insurance, and deductibles on the day that services are performed.
Many patients also carry secondary insurance coverage. Secondary insurance will usually pay out-of-pocket expenses for the balance remaining after the claim has been processed by primary insurance. We will file to both primary and secondary insurance on your behalf if we participate with your plans. Please be aware that cosmetic services are not considered medically necessary and the charges for these services will not be filed with your insurance carrier.
